All first time patients are asked to bring the following:
- All pertinent medical records and test results
- A list of current medications and supplements both prescription and over-the-counter
- Insurance card and photo identification
- Co-pay
The office accepts Visa, Discover, MasterCard, American Express and cash.
As a courtesy to patients who may have an immediate need to schedule, we ask that cancellations be called into the office at least 24 hours in advance. Should you fail to cancel, you may be billed for the scheduled time.
